I have done all the above and I have read the help "Business Card and Label
Printing" and "Mass Mailing and Address Label Printing" many times, and I
cannot figure out what I am making wrong.

The merge file holds about 1300 addresses, to save paper when testing I have
only been printing up to 45 (starting at 1, ending at 45). What I get on
paper is one label on each page up in the left corner, the 34'th address
comes in the same position on the following page, but the 32 addresses in
between are all missing.
No error report. I have also tried to print 2 across and 6 down the only
difference is that on the second page is the 13'th address, on the 3'rd
page only the 25'th etc (I thought that if the labels were too large and 33
would not fit on the page)

Seems to match what I'm doing to get my labels printed.
Have you set the "Imposition" tab "Option" selection to "Repeating" ?

What I had missed follows explained by Robert Williams at Total Amiga:

I did only have a text frame of the right size on an A4 size page.

Also the EURO-characters are at the right places in my document, that's the way described by Tim Doty:
