It looks like the docs are out of date (http://pagestream.org/?action=Documents&id=642) but IIRC you hold down shift. Of course, you can always go to Layout/Document Setup menu after the document opens.

Could you be more specific about what doesn't work? Holding down the
shift key when selecting New Document should bring up the old dialog box
asking for initial master page size etc. If you simply create a document
based on the default sizes and want to change it after the fact, choose
Master Page Setup from the Layout menu.
