In case someone else has seen this and can add to it:

Once in 4.1.6.13 and again (just now) in 4.1.6.14 I have had a bug that I was
unable to otherwise reproduce. PgS was already running, I don't remember for
sure what else I had done. No other document windows open, click New Document
button in the PgS Navigator. Requestor appears, click OK (or Finish or
whatever it says). The requestor disappears, but nothing else happens. Repeat
and it works.

The first time this happened I had set various parameters (named the document,
altered the defaults for margins, etc.), this time I just took all defaults.
Doing it either way has worked every other time I have tried.
